免費生前醫療指示文件講座

亞美健康關懷服務將與華人活動中心將於二月十六日星期六下午兩點聯合舉辦免費生前醫療指示文件講座，為大家解釋並協助大家填寫兩份生前醫療指示文件，兩份文件包括 a.生前醫療意願書 (Living Will or Directive to Physicians and Family or Surrogates)，這份文件是讓醫院、醫生、或親人知道當你不冶時，你是否要繼續靠人工系統來維持個人的生命。 b. 醫療委託書( Medical Power of Attorney)，指定一個或數個醫療代理人，代理人(委託人）可以為你在你無法為自己做醫療決定時，替你做決定。這兩份文件都必須在個人神智清醒時準備好、以便不時之需。講座過後將會開放Q&A 時間，也提供表格檢閱服務。

Free Living Will and Medical Power of Attorney Seminar

2/16/2019 2:00 pm AACCS and DCCC will hold a free seminar. The seminar will explain and assist you to fill out two advance directives. After the seminar, there would be a Q&A section and a form review service will be provided.

Advance directives are legal documents that allow you to convey your decisions about end-of-life care ahead of time. They provide a way for you to communicate your wishes to family, friends and health care professionals, and to avoid confusion later on.

AACCS will provide free services to explain and assist participants to complete the following advance directive documents:

Directive to Physicians and Family or Surrogates

This form is designed to help you communicate your wishes about medical treatment at some time in the future when you are unable to make your wishes known because of illness or injury.

Medical Power of Attorney

Except to the extent you state otherwise, this document gives the person you name as your agent the authority to make any and all health care decisions for you in accordance with your wishes, including your religious and moral beliefs, when you are no longer capable of making them yourself.
